We had that exact situation. Our son bypassed primary and then had mod quad at 10 months and triangle tilt (acromioplasty) at 2 years old.
Danny had mod quad because of very strong internal rotation - the muscles were so tight. He had acromio because his shoulder became dislocated and his winging was bad.
