Parker has never worn an extension splint. It seems I read about it long ago, but no one has ever suggested it for Parker? It sounds great! I wonder why nobody has ever brought it up? Thanks!

I've seen some BPI kids with severe bowing of the upper or lower due to muscle imbalances. Is this what you are talking about? In that case, an osteotomy, which just involves a resetting of the bone into a new rotation at one discrete point, wouldn't do anything to correct the bow in the arm. That may be why the Ilizarov was recommended and not an osteotomy.

Hello
I had a consultation with Dr. Guggenheim and he told me I was not a candidate for the illizarov. However, I know a gal who had it done with Dr. Guggenheim. It is quite the contraption. If you would like I can get a hold of her and have her e mail you information. Her name is Deanna from Alabama. I haven't talked to her in a month or so but can get a hold of her for you if you would like. She was the first adult I think to have it done.
my email is marko@centurytel.net

I believe that on Dr. Nath's website there is a picture of a boy with this procedure, and of the brace that is required for wear. Camille B.

Hello,

My BPI son is now 7 as well. When he was 3 he had about a 40% contracture at the elbow. The contracture was caused by muscle imbalance. He has very good bicept return but no triceps. We tried casting, splinting, it just was a temp fix. At age 3, we did bicept lenthening surgery. That was an immediate fix. It has stayed pretty much. At age 7, he does now have a slight less then 10 degree contracture at elbow which we are doing dynsplint at night. I think some of the bending is also preference. He tends to hold his arm that way too. Dr. Nath was the one who recommended the bicept at age 3 and Dr. Shenaq actually did the surgery. Back then they were both together and on surgery day either one of them could do it. :) Dr. Nath does do the Bicept Tendon Lengthening too. Not sure if you were aware of this. Best of luck with your decision.

The International Center for Limb Lengthening (ICLL) is internationally recognized as the most experienced center for limb lengthening and reconstruction in the world, committed to providing the most comprehensive and technologically advanced treatments available for children and adults with upper and lower limb length discrepancies, short stature, limb deformities, nonunions, bone defects, bone infections, joint contractures, and arthritis. In addition, the ICLL has become a major referral center for the Ponseti method of clubfoot treatment and distraction treatment of Perthes disease, two new methods for the treatment of two common pediatric disorders. The ICLL also offers a state-of-the-art gait laboratory for motion and function analysis and other specialized studies. With an international reputation for excellence, the Center's physicians have treated patients from all 50 United States and more than 40 countries from six continents around the world.

The ICLL, part of the Rubin Institute for Advanced Orthopedics, opened at Sinai Hospital of Baltimore in July 2001 in a brand new six-story facility designed by the Center's co-founders, Drs. Dror Paley, John E. Herzenberg, and Michael Mont. Drs. Paley and Herzenberg were previously part of the Maryland Center for Limb Lengthening & Reconstruction at Kernan Hospital for 14 and 10 years, respectively, and were the first team in the United States to dedicate their practices to limb lengthening and deformity reconstruction. To expand their facility and offer new and improved clinical and research services, they moved their operation to the new Rubin Institute for Advanced Orthopedics and added a third limb lengthening surgeon, Dr. Janet Conway. By teaming with Dr. Mont, the new Center is able to offer state-of-the-art joint replacement and joint preservation techniques that were not available at the MCLLR.

The ICLL is part of Sinai's Rubin Institute for Advanced Orthopedics (RIAO), which provides state-of-the-art orthopedic expertise in various subspecialties of orthopedic surgery, such as total joint replacement and treatments for avascular necrosis. The ICLL also has access to the full array of medical services available at Sinai Hospital, a 428-bed community teaching hospital with residency training programs in pediatrics, internal medicine, surgery, obstetrics and gynecology, and rehabilitation medicine. Sinai Hospital offers all the benefits of a modern teaching hospital, including up-to-date state-of-the-art anesthesia, pain management, radiology, and emergency and intensive care facilities, with the warmth and feel of a community hospital.

Dr. Nath also recommended my son consider getting limb lengthening for his BPI arm and recommended that we see Dr. Paley in Baltimore. My son is almost 12 and wants righty to look like his uninjured arm so we are going to check it out. I hate to have a surgery that doesn't improve function so I'm not sure how I feel about it. Has anyone had or have considered limb lengthening for the BPI arm??

This has been recommended for my daughter as well, by Dr. Nath. She is 14. He suggested Dr.GUGGENHEIM or Dr. Pauly out of Baltimore. Dr. Nath does not do this surgery. I would like to be in contact with anyone who is considering this, and or has had it done.

Thanks in advance,
Brenda
