Do we write our Last Name, injured childs name, or do I just sign it?

I'm confused!! It just says Name.

You sign it on behalf of your family.

My last name is Grimes, my daughter's(lbpi) last name is Walker...so I signed "Grimes-Walker".

Yes that would be fine, we just need a signature of a parent saying it is ok that pictures we take of any member of your family, we can use those photos in future promotional material or for publication, like maybe on our website, in a brochure, or in a future issue of Outreach.
